During that time my wife had cancer. She had surgery and chemo therapy and is now considered cured after five years of treatment.

Start funding a healthcare savings account and raise the insurance deductible as the account grows.
When you pay directly for most of your healthcare costs, you will be motivated to negotiate fees. You will be surprised how flexible prices are.
