My wife and I have paid for our own health insurance for 17 years.

During that time my wife had cancer. She had surgery and chemo therapy and is now considered cured after five years of treatment.
Even with the high cost of cancer treatment, we would be tens of thousands of dollars ahead if we had never purchased health care insurance, and instead, put the money in a healthcare savings account.
Start funding a healthcare savings account and raise the insurance deductible as the account grows.
When you pay directly for most of your healthcare costs, you will be motivated to negotiate fees. You will be surprised how flexible prices are.
